Coloured copy of your passport *
- Valid for minimum period of 6 months at the time of travel - Single name passports are not accepted
Colored Photo 4 cm x 6 cm in Yellow Background *
Color passport photo size 4 x 6, six (6) copies with a yellow background. The photo must show the applicant wearing polite attire with a collared shirt. The photo should not include facial accessories, the face must be clearly visible, and for applicants wearing a hijab, the face must still be fully visible.
Letter of Attorney for SKCK *

Letter of request from sponsor *
Letter of request from the sponsor, company, or institution that employs, engages, or is responsible for the foreign national (WNA).
Photocopy of the Expatriate Employment Permit (IMTA) issued by KEMENAKER RI (Optional) *
Attach a photocopy of the Expatriate Employment Permit (IMTA) issued by the Ministry of Manpower of the Republic of Indonesia (KEMENAKER RI).
Birth certificate (Optional) *
Birth certificate showing full name, date, and place of birth.
Family certificate (Optional) *
Certificate that shows that you & your dependent are family from your country
Sponsor ID *
A scan of the company's/person's sponsor ID.
Marriage Certificate (Optional) *
Official proof of your marital status. You can obtain it from your local civil registry office.
